Yuan-Rung’s Innovation in Home-Based Care: The Integration of Traditional Chinese Medicine into a 4-Dimensional Care Model

Acronym: “IC4D-TM” (Integration of Chinese 4-Dimensional Traditional Medicine)

In recent decades, the traditional Chinese medicine was not included in home-care medical system. For people who cannot go out, it is not easy to obtain TCM resources for medical treatment. Therefore, we launched the “Yuan-Rung 4-dimension Home-based Medical Integration Program (referred to as: Yuan-Rung 4D Home Care Team)”, dedicated to improving the quality of care for the disabled. 
Before the NHI includes the TCM into home-care system, we took the lead in participating in the welfare resources in Changhua County, and introduced “4D multi-axis holistic care”. The effect showed 60% of the cases has over 1-year-free of emergency department visits, and the average number of outpatient and emergency medical visits is lower than the average of the Chinese (including healthy adults). 
After operation and optimization, the service popularity is from Yuanlin stepped into the “Zhanglian Medical Group”. With the help of mobile cloud integration, it can achieve comprehensive matching without missing connections, and it has the effects of high accessibility, high immediacy, high popularity. We have also been invited as the director or give speeches to share practical experience. In the future, we will develop an intelligent case management model, apply for the main training institute of TCM and family medicine, publish results in journals, inherit and promote the advantages of our team, and benefit more people by the model combining TCM to maintain a more elegant and healthier lifespan.
